West Bradford Youth Athletics - Background Screenings

In this session, you will be asked information which will be used to conduct a background screening by the National Center for Safety Initiatives, LLC (NCSI). Please read through each page carefully to ensure that all information is provided accurately.

In accordance with Pennsylvania law (23 Pa. C.S. § 6344.2), West Bradford Youth Athletics mandates the following clearances before one's background screening results will be reviewed by West Bradford Youth Athletics.

1. Pennsylvania Child Abuse History Clearances (CY113)
2. Pennsylvania (State Police) Criminal Record Checks for Employment (SP4-164) / Pennsylvania (State Police) Criminal Record Checks for Volunteers (SP4-164A)
3. Federal Bureau of Investigations (FBI) Criminal Background Check or Disclosure Statement Application for Volunteers.
